Job Description & RequirementsRN Case ManagerStartDate: 2/10/2025 Pay Rate: $2000.00 - $2700.00POSITION SUMMARY - RN Case ManagerPOSITION DUTIES - Acute Care Case Management in UR, DCP and Transitions of Care -Home Health and Skilled Nursing Facility set ups.MINIMUM REQUIRED QUALIFICATIONS - FL/compact RN license; BLS; 2 years acute hospital case management experience; DCP Assessment Skills; Experience with Meditech, MIDAS, and InterQual.PREFERRED QUALIFICATIONS - CCM or ACM.LENGTH OF ASSIGNMENT - 13 weeksSHIFT / HOURS PER WEEK - 9:00AM - 5:30PM with weekend rotationSYSTEMS - Meditech/Midas/InterqualSTART DATE - ASAPFacility LocationSanford is located in the central region of Florida and used to be home to various Indian tribes who inhabited the shores of Lake Monroe.
