Job Description:

Baptist Health is looking to add a Social Worker II to our Social Services team at Baptist Medical Center located in Jacksonville, FL. This is a PRN opportunity working primarily weekend shifts as they are needed within the department. As a Social Worker with Baptist, your primary goal is to enhance patient quality outcomes while maintaining ongoing communication with the patient, family, medical staff, caregivers, or third-party payors to set priorities, plan, organize, and implement care to decrease the amount of readmissions.As a Social Worker II, you will be responsible for:

• High level screening, assessing, and planning for an identified high-risk group of patients in the Emergency Department.
• Collaborating with the attending and resident staff as well as nursing, ancillary, professional, and technical staff, to coordinate the delivery of high-quality, individualized, cost-effective patient care.
• Utilizing Utilization Review criteria along with admission and discharge criteria to appropriately screen patients for admission or discharge.Education Requirement: A Master's in Social Work is required (MSW).Education RequiredMaster's DegreeEducation PreferredDoctorateExperience
  • 1-2 years Counseling Experience Required
  • 1-2 years Experience in Child Welfare Services Required
