This position also comes with a $5,000 Sign-On Bonus
Job Function: Provides and directs Physical Therapy services to
residents, including assessment, treatment, program planning and
implementation, related documentation, and communication. Functions
under physicians' orders.
Responsibilities Include:
--- Provides Physical Therapy services to residents and initiates
referrals when appropriate and in accordance with department
procedures.
--- Conduct screenings of residents at regular intervals to
determine the need for intervention/treatment.
--- Evaluates residents to obtain data necessary for treatment
planning and implementation.
--- Conduct specialized evaluations as indicated.
--- Adheres to established confidentiality standards.
--- Implements and conducts treatment as outlined in treatment
plan.
--- Follows management direction.
--- Performs other duties as assigned.
Education and Experience:
--- Bachelor of Science in Physical Therapy from an accredited
program.
--- Successful completion of National Certification Examination for
Registered Physical Therapist.
--- Current state licensure or license eligible.
--- 0-3 years of long-term care experience.
